Dr. Vijay Janagama is a pioneering force in transition care, wellness, and rehabilitation, with over 20 years of expertise spanning healthcare technology and patient-centered care. As a Health Promotion Specialist, Diabetologist, and Certified Wellness Practitioner, he played a pivotal role in conceptualizing India’s first transition care facility, SuVitas Holistic Healthcare, in 2015.
A key architect of globally accredited protocol-based rehabilitation models, Dr. Vijay spearheaded the development of SuVitas’ Transition Care Plan, ensuring high standards of post-hospitalization care. As Founding Medical Director, he led multidisciplinary care teams, driving research, innovation, and patient-centric treatment approaches.
His contributions to post-hospitalization and rehabilitation research have been recognized on global platforms, including the World Stroke Congress, World Congress of Neurology, and World Congress of Neuro Rehabilitation. He is also a passionate advocate for preventive healthcare, serving as Director, Health Promotion at Prevent NCD Foundation, where he has led impactful community health awareness initiatives under the HAPPI (Health Awareness Promotion – Project India) program.
In addition to his work in clinical care, Dr. Vijay is one of the founders of YAGNA, a nonprofit organization dedicated to health, education, and leadership development. In his current role as Head – Concept & Clinical Excellence at HCAH India, he focuses on care delivery, quality accreditation, capacity building, healthcare standardization, innovation, and leveraging technology to enhance patient outcomes and experiences.
